To schedule an appointment please email the following information to appts@thespiritchrysalis.com with "Appointment Request" in the subject line:

1. Full Name
2. Phone number & Time Zone
3. Type of Session Desired:

         Office Reading
         Zoom Reading
         Phone Reading
         Energy Session (2hrs) $200
         Pet Reading (20 Mins) $45

4. For Readings, Length of Session:

          30 Mins - $90
          60 Mins - $175

5. Your availability within Frank’s hours (listed in Central Time):
   
     Monday: 3p – 7p
     Tuesday, Wednesday, Thursday: 11a – 5p
     Friday: 11a – 4p
     Saturday: 11a – 3p;  One Saturday per month; Date varies by month. These appointments
                                      fill quickly.
    
6. If a guest will be attending with you, please provide their name/s.
     Max of two guests in addition to you. $20 per guest.

7. Preferred method of payment:

           Debit or Credit card by PayPal Invoicing
           Check or Money order via USPS
